ADDITIONAL ACKNOWLEDGMENTS AND AUTHORIZATIONS FOR BACKGROUND CHECK

As disclosed and authorized in the separate Fair Credit Reporting Act Disclosure and Authorization document, your employer (the “Company”) and Amazon.com, Inc. or its subsidiaries or affiliates (“Amazon”) may procure a consumer report, also known as a background check, about you in connection with your possible assignment to provide delivery services to Amazon on the Company’s behalf (the “engagement”). In connection with that background check, you acknowledge as follows:

  • You understand and agree that, as allowed by law, Amazon can order additional background checks about you without your further authorization (1) any time during the engagement, and (2) from the background check provider or “consumer reporting agency” (“CRA”) of Amazon’s choice.

  • For these purposes, and subject to all laws protecting your informational privacy, by accepting this Additional Acknowledgments and Authorizations for Background Check, you authorize the following, among others, to disclose to the CRAs any information needed for the background check: your past or present employers, schools, and law enforcement and other government agencies, including motor vehicle record agencies.

MINNESOTA: You may contact Amazon to determine which CRA conducted your background check. You have the right to submit a written request to the CRA for a complete and accurate disclosure of the nature and scope of any consumer report that was ordered about you. The CRA must provide you with the disclosure within five business days after its receipt of your request or when the report was ordered, whichever date is later.
NEW YORK: You have the right to submit a written request to Amazon to know whether Amazon ordered a consumer report or an investigative consumer report about you and, if so, the name and address of the CRA that provided the report. You also have the right to contact the CRA to receive a copy of any investigative consumer report ordered about you.

IDENTITY VERIFICATION AND BIOMETRIC CONSENT

From time to time, Amazon may require you to submit photographs to confirm your identity. Using your photograph, we may create a facial scan or similar biometric identifier, which we refer to as “Biometric Information,” and collect, store, and use Biometric Information from your submitted photographs (including those we already have on file) and your driver’s license or government-issued ID. As a condition of delivering Amazon packages using the Amazon delivery application, you consent to allow Amazon to verify your identity using your photograph and Biometric Information derived from your photograph and share your photograph with our customers for identification purposes. Amazon generally retains a user’s Biometric Information only for as long as it takes to complete the verification of your identity, but may retain Biometric Information for up to 30 days after it is generated. VEHICLE TECHNOLOGY AND BIOMETRIC CONSENT

At Amazon we are continuously evaluating technologies that make it easier for drivers (referred to in this consent as “Delivery Associates”) to deliver packages safely and securely, as well as improve the quality of the delivery experience for Delivery Associates and customers. The vehicles you operate while making deliveries may come equipped with technology such as telematics devices that collect GPS and navigation-related information, cameras with video and photograph capabilities, and sensors (the “Technology”). The vehicles are video-monitored by cameras that are both internal and external and that operate while the ignition is on and for up to 20 minutes after the ignition is turned off. With certain Technology, you may have the ability to deactivate a camera more quickly after the ignition is turned off, and can do so by following the instructions available from your employer. The Technology also tracks vehicle location and movement, including miles driven, speed, acceleration, braking, turns, and following distance.
This Technology allows Amazon, its affiliates, Technology providers and contractors, and your employer to gather information that helps Amazon and your employer improve the safety and quality of the delivery experience and to protect Delivery Associates, including to alert them when driving distractions are presented. Amazon may also use certain Technology that processes Biometric Information, including on-board safety camera technology which collects your photograph for the purpose of confirming your identity and connecting you to your driver account. Using your photograph, this Technology, may create Biometric Information, and collect, store and use Biometric Information from such photographs. Using this Technology, Amazon generally retains a user’s Biometric Information only for as long as it takes to complete the verification of your identity and associating the Technology to your driver account, but may retain Biometric Information for up to 30 days after it is generated.
